Family Support Specialist – Healthy Families (Full-time)
Bond County Health Department is looking for a full-time Family Support Specialist in their Healthy Families Illinois Program. The Family Support Specialist provides high-quality and comprehensive services to children and families enrolled in the Healthy Families Program. Services are available to expecting families and their children ages 0 – 3 years of age and provided in homes, shelters, transitional housing, or wherever a family calls home.
Requirements
- Associate’s degree in Human Services, Child Development, Psychology, Education, or related field required, or two years of related experience providing services to children and families required.
- Bachelor’s degree in the above-mentioned fields preferred.
- Must understand the social and economic challenges of the service area and be familiar with the community resources available for families in Bond County and parts of Madison County.
- Valid driver’s license and access to an insured vehicle required.
- Ability to travel to Springfield for multiple trainings is a must.

Registered Nurse (Part-Time)
The Bond County Health Department has an opening for a Registered Nurse to work in the programs of Maternal and Child Health (MCH), Women Infants & Children (WIC), Family Planning, and Immunizations. This is a part-time 28 hours per week position.
Responsibilities
- Assesses and monitors pregnant women throughout pregnancy as well as infants and children through the stages of physical, cognitive, language, and social-emotional development.
- Provides intervention and support to promote the health and well-being of children and their families.
- Provides families with information and resources to assist them in obtaining their maximum health potential.
- Evaluates the nutritional status of individuals and provides supplemental WIC benefits based on needs identified.
- Provides daily office duties per department policies and procedures: immunizations/TB/STD/ Family Planning/ BP/lab draws as well as other services requested by residents of the community that is within the nurse’s scope of practice and the facility’s capabilities.

Social Worker – Home Health/Hospice (PRN)

Flexible work hours based on approximately 10 hours per month. Employment available as a part-time staff member or independent contractor.
Position Responsibilities Include
- As part of the Hospice Team, the social worker will assess and care for social needs as they relate to the patient’s illness.
- Links patients and their families to available community resources when needed.
- Participates in the Hospice Interdisciplinary Group meetings to assist the team in maintaining an individualized comprehensive plan of care.
Requirements
- Master’s degree in social work from a program accredited by the Council on Social Work Education (CSWE) required and licensed as a Social Worker (LSW) or Clinical Social Worker (LCSW) in the state of Illinois.
- One year of experience in a health care setting is preferred.
Positions Available in Prairie Counseling
Prairie Counseling is a small community mental health center located within the Bond County Health Department. This is a fast-paced, energetic, comprehensive team of 12 who pride themselves in providing an array of mental health and substance use treatments for clients. The clinic is known for top-notch service and creative ways of “going the extra mile”.
Mental Health Professional (LPC, LCSW, LMFT, LCPC)
Prairie Counseling Center has an opening for a therapist. Licensure is preferred but new grads are encouraged to apply. Candidates must be friendly, outgoing, honest, dependable, and organized.

Position Roles/Responsibilities/Accountabilities
- Comfortable with utilizing technology at all points of the day including telehealth software, video communication, and internal communication tools
- Ability to utilize comprehensive assessments
- Knowledge of mental health and/or substance abuse diagnosis
- Facilitate individual and group therapy sessions
- Formulate treatment plans
- Experience working in partnership with clients to achieve goals
- Monitor progress throughout the treatment.
- Work collaboratively with a treatment team and attend Clinical Consultation meetings to collaborate on intervention plans with other mental health specialists.
- For counselors that are not fully licensed, work towards licensure and attend clinical supervision as needed.
- Provide case management services for patients.
- Communicate with outside agencies on behalf of your patients.
- Provide clinical documentation and letters for patients as needed.
- Ensure all patients are scheduled on a regular basis.
- Ability to complete all documentation in a timely manner.
- Must be willing to work in community outreach as needed

Position Qualifications
- Bachelor’s degree (master’s preferred) in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or family therapy, or related field; or
- Certificate of psychiatric rehabilitation from a DHS-approved program plus a high school diploma plus 2 years’ experience in providing mental health services; or
- Recovery support specialist certified and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association Inc. plus one year experience in providing mental health services.
- A supervised internship in a mental health setting counts toward the experience in providing mental health services.
Mental Health Professional (Full-time)

Minimum Qualifications
- An individual possessing at least a master’s degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or family therapy or related field who has successfully completed a practicum or internship that included a minimum of 1000 hours of supervised direct service or who has one year of clinical experience; or
- A registered nurse (RN) licensed under the Nurse Practice Act [225 ILCS 65] with at least one year of clinical experience in a mental health setting or who possesses a master’s degree in psychiatric nursing; or
- An occupational therapist (OT) licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with at least one year of clinical experience in a mental health setting.

Crisis Counselor – Prairie Counseling Center (On Call – As Needed)
Bond County Health Department is looking for a crisis counselor to join their team in the Prairie Counseling Center. The crisis counselor completes assessments along with recommendations for stabilization and support for people in crisis. Must possess a basic understanding of mental health crisis and be able to work with individuals and other health care providers. Services may include emotional support, individual safety planning, and voluntary/involuntary placement when needed.
Minimum Qualifications
- Crisis Counselors must at a minimum meet Medicaid 132 requirements for a Mental Health Professional (MHP) OR possess a Medicaid approved Bachelor’s degree OR possess a Bachelor’s degree in any other field with two years of supervised clinical experience in a mental health setting.
- Other requirements include minimum of 21 years of age, valid driver’s license, and an acceptable driving record.
