Always Available

Care is available seven days a week. If a problem should arise, a registered nurse is on call 24 hours a day to offer advice over the phone and, if necessary, to make a home visit for current patients and referrals.

Bond County Health Department Home Health Care (Visiting Nurses) began in 1966 and has advanced through the years to meet the needs of the community and neighboring counties.

Our Care Philosophy

Health care in the home is comprehensive yet specific to the patient which contributes to the quality of life and may decrease recovery time. Bond County Home Health provides an alternative for patients who need skilled care, but do not require 24-hour supervision.

Services are provided to patients under the direction of the patient’s doctor. Each patient has an individualized plan of care to acquire the best medical, physical, and psychosocial outcomes. The plan of care includes your physician, one-on-one care, family/caregiver assistance, patient education, disease intervention, and promotes independency to your health care needs.

The goal of Home Health is to provide skilled quality patient care in the home setting. As patient insurance and Medicare/Medicaid providers require hospitals to reduce their hospital stays, a number of patients returning home have ongoing needs. Home Health care is a valuable alternative to long hospital stays or nursing home placement.

Our Services

Skilled Nursing Care

Complete assessments, wound care, pain management, IV therapy, catheter care, diabetic care, medication management, and patient education by RNs and LPNs.

Home Health Aide

Personal care including bathing, shampooing, oral hygiene, skin care, positioning, transferring, vital signs monitoring, and assistance with daily living activities.

Pediatric Care

Synagis injection program, failure to thrive monitoring, well-baby assessments, and referrals to WIC, HMHK, and appropriate community services.

Medical Social Worker

Long-term planning, community resource coordination, patient advocacy, financial resource assistance, and emotional support for patients and families.

Physical Therapy

Home exercise programs, mobility restoration, therapeutic exercises, muscle strengthening, pain relief, gait training, and transfer training.

Speech Therapy

Voice disorders, speech articulation, non-verbal communication, swallowing difficulties, and stroke rehabilitation services.

Occupational Therapy

Fine motor skills recovery, personal care techniques, energy conservation, assistive device assessment, and upper extremity strengthening.

Private Duty Services

Alternative care for patients who don’t meet Medicare/Medicaid requirements. Services by licensed RNs, LPNs, and certified CNAs under physician plan of care.

Frequently Asked Questions

How is a Referral Made?

A referral may be made by family, case managers, social workers, physicians, or patients themselves. Physician orders will be required prior to admission for Home Health services. Referrals may be made prior to entering a hospital for elective or anticipated treatment by calling (618) 664-5020. After hours, weekends and holidays, this number is also effective in reaching the nurse on-call.

Who is Eligible for Services?

Bond County Home Health and Hospice is available to persons who meet payment source criteria, are in need of skilled care, and reside within Bond, Clinton, Fayette, Madison, and Montgomery Counties.

How is Home Care Paid For?

Home care is paid for by a variety of sources including Medicare, Medicaid, Older Americans Act, private health insurance, Veterans’ Administration, Worker’s Compensation, HMOs, PPOs, CHAMPUS, social organizations, and patient/private pay.

Accepted Insurance: Blue Cross Blue Shield of Illinois, GHP (Group Health Plan), Aetna, UHC (United Health Care)/Medicare Advantage, Medicare, and Medicaid (Meridian).

Can I Make a Donation?

When families, individuals, and patients name the Bond County Health Department to receive contributions and memorials, it is a tribute not only to the patient but to the Home Health staff. Memorials and donations are greatly appreciated and tax deductible. No individual employee may accept a gift of value for services.
