Registered Nurse – Clinic (Full-Time)

The Bond County Health Department has an opening for a Registered Nurse to work in the clinic.  This is a full-time position at 35-hours per week. 

Responsibilities

  • Assesses and monitors pregnant women throughout pregnancy as well as infants and children through the stages of physical, cognitive, language and social-emotional development.
  • Provides intervention and support to promote the health and well-being of children and their families. 
  • Provides families with information and resources to assist them in obtaining their maximum health potential. 
  • Evaluates nutritional status of individuals and provides supplemental WIC benefits based on needs identified.
  • Administers vaccines and draws lab tests.

Requirements

Associate’s or Bachelor’s degree from an accredited school of nursing

Active Illinois Registered Nurse License

Current BLS certification

Mental Health Counselor (Full-Time)

Prairie Counseling Center, a service of the Bond County Health Department, has an opening on their team of professionals for a mental health counselor.  Providing services to a population with diverse needs, the counselor will assist clients in reaching their desired goals.

Job Duties

  • Completes clinical screening to assess the individual needs of clients.
  • In collaboration with the client, develops and facilitates a plan for treatment and monitors progress.
  • Maintains working relationship with potential referral sources.
  • Participates in Prairie Counseling’s overall program planning, development, and in clinical staff meetings
  • Keeps accurate, up-to-date client records, reports, and provides required statistics.

Education/Qualifications

  • Minimum Requirement: Meets QMHP requirements established in Rule 132
  • Some knowledge of substance use desired
  • Preferred – LCSW or LCPC or someone working towards licensure

 

Hours are flexible and may include late afternoon or early evening hours.  Weekend and holidays are not typical. 

Physical Therapist – Home Health (Full Time)

Licensed Physical Therapist needed for full-time work in our Home Health Department.  Providing therapy and rehabilitation services to patients in the home, the ideal candidate is a compassionate healer who is passionate about their patients and committed to helping them recover their independence and mobility.  Home Health services are provided to patients in Bond, Clinton, Fayette, Madison, & Montgomery Counties.

Essential Duties

  • Working with the patient’s medical provider, plans a physical therapy program to restore function, relieve pain, prevent disability, and improve or maintain muscle function.
  • Develops and administers the patient’s care plan and monitors progress.
  • Oversees the work of the physical therapy assistant as well as collaborates with other health team personnel on the patient’s treatment, response, and progress.

Education/Qualifications

  • Current Illinois physical therapy license
  • Experience in the home health field preferred.

Physical Therapy Assistant – Home Health (Full Time)

Bond County Health Department is seeking a Physical Therapy Assistant to join our Home Health Team.  Working under the direction of the physical therapist, the physical therapy assistant implements the individualized plan of care to help patients restore function, relieve pain, and prevent disability following disease, injury or surgery.  Home Health services are provided to patients in Bond, Clinton, Fayette, Madison, & Montgomery Counties.

Essential Duties

  • Provides therapeutic exercises and activities to help patient’s achieve movement goals, and regain or maintain their independence.
  • Monitors patient’s progress and collaborates with other members of the home health team for improvements.
  • Educates the patient as well as their families on ways to incorporate restorative practices to improve the patient’s everyday life.

Education/Qualifications

  • Current Illinois physical therapy assistant license
  • Excellent interpersonal skills and a passion for assisting others reach their goals
  • Experience in the home health field preferred.

Social Worker – Home Health/Hospice (PRN)

Flexible opportunity for optimal work-life balance. Serving the community for over 50 years, our established department is part of the Bond County Health Department in Greenville and cares for patients in Bond, Clinton, Fayette, Madison, & Montgomery Counties. Flexible work hours based on approximately 10 hours per month. Employment available as a part-time staff member or independent contractor.

Position Responsibilities Include:

  • As part of the Hospice Team, the social worker will assess and care for social needs as they relate to the patient’s illness.
  • Links patients and their families to available community resources when needed.
  • Participates in the Hospice Interdisciplinary Group meetings to assist the team in maintaining an individualized, comprehensive plan of care.

Master’s degree in social work from a program accredited by the Council on Social Work Education (CSWE) required and licensed as a Social Worker (LSW) or Clinical Social Worker (LCSW) in the state of Illinois. One year of experience in a health care setting is preferred.

Crisis Counselor – Prairie Counseling Center (On Call – As Needed)

Bond County Health Department is looking for a crisis counselor to join their team in the Prairie Counseling Center.  The crisis counselor completes assessments along with recommendations for stabilization and support for people in crisis.  Must possess a basic understanding of mental health crisis and be able to work with individuals and other health care providers.  Services may include emotional support, individual safety planning, and voluntary/involuntary placement when needed.

Crisis Counselors must, at a minimum, meet Medicaid 132 requirements for a Mental Health Professional (MHP) OR possess a Medicaid approved Bachelor’s degree, OR possess a Bachelor’s degree in any other field with two years of supervised clinical experience in a mental health setting. Other requirements include minimum of 21 years of age, valid driver’s license and an acceptable driving record.
