Health Care, (Visiting Nurses) began in 1966 and has advanced through the years
to meet the needs of the community and neighboring counties. Counties of
licensing include: Bond, Clinton, Fayette, Madison, and Montgomery.

Health care in the home is
comprehensive yet specific to the patient which contributes to the quality of
life of our patients. Recovery time is
decreased while cost is minimized.
Each patient has an individualized
plan of care to acquire the best medical, physical, and psychosocial
outcomes. The plan of care includes your
physician, one-on-one care, family/caregiver assistance, patient education,
disease intervention, and promotes independency to your health care needs.
The goal of home health is to reduce
recurrent hospitalizations and reduce complications. Hospital stays are becoming shorter and
shorter. A number of patients returning
home from the hospital have ongoing needs and without home health care,
recovery may be longer. Home healthcare
is a valuable alternative to long hospital stays or nursing home
placement. Patients and family
compliance enhances the care we give.
How is a Referral Made?
A referral may be made by family,
case managers, social workers, physicians, or patients themselves. Physician orders will be required prior to
admission for Home Health services.
Referrals may be made prior to entering a hospital for elective or
anticipated treatment by calling the office at 618-664-5020. After hours, weekends and Holidays this
number is also effective in reaching the nurse on-call.
Private Duty Services
Home Nursing may be an alternative to care
when a patient wants to remain at home but does not meet the requirements for
Medicare, Medicaid or insurance reimbursement.
This may be due to ongoing problems that are chronic or the patient may
not be homebound.
Private duty service, under the
license, will be provided by licensed registered or practical nurses. Certified CNA's will also be utilized.
Each service provided will be under a
physician plan of care, a contract between the patient and the Home Nursing
program will be signed, and the patient/family will pay for the services.
Home Services is another fee-based service
that provides non-medical services to residents in their home such as personal
care. Additional services may be added
at a later date. Please check with the
Home Service department with any questions.

How is Homecare Paid For?
Home care is paid for by a variety of
sources including Medicare, Medicaid, Older Americans Act, private
health insurance, Veterans' Administration, Worker's Compensation, some Health
Maintenance Organizations (HMOs), Preferred Provider Organizations (PPOs),
CHAMPUS, social organizations and patient/private pay.

24 Hour On-Call Availability
Care is available seven days a
week. If a problem should arise, a
registered nurse is on call 24 hours a day to offer advise over the phone and,
if necessary, to make a home visit for current patients and referrals.

Eligibility
Bond County Home Health and Hospice
is available to persons who meet payment source criteria, in need of skilled
care, and reside within Bond, Clinton, Fayette, Madison, and Montgomery
Counties.

of specimens) with additional skill.

care of catheter.

Home Health Aide Care

Personal care, (i.e., bathing, shampooing, oral hygiene
care, and skin care).

Body mechanics, (i.e., positioning, transferring,
assistance with walker and crutches, range of motion).

General procedures, (i.e., monitoring vital signs, enemas,
douches, simple dressings).
·
Reporting changes and needs to nursing.
·
Instructional/Reinforcement to independency with ADL's.
Medical Social Worker
·
Long term planning for medical intervention.
·
Community resource person.
·
Patients Advocate.
·
Assist with obtaining financial resources.
·
Provides emotional support to patient and family.
Occupational Therapy
·
Helps patients regain fine motor skills, (i.e., writing,
personal care, energy conservation techniques).
·
Assesses patients needs for assistive devices for
activities of daily living.
·
Upper extremity strengthening/splinting.
Speech Therapy Services
Care is directed toward:
·
Voice disorder.
·
Speech Articulation.
·
Non-verbal communication.
·
Swallowing difficulties.
·
Stroke rehabilitation.
Physical Therapy Services
·
Home Exercise Program.
·
Helps patients regain mobility by therapeutic
exercises/muscle strengthening.
·
Helps patients increase strength and relieve pain.
·
Muscle re-education skills.
·
Gait training, transfer training.
Donations and Memorials
When families, individuals, and
patients name the Bond County Health Department to receive contributions and
memorials, it is a tribute not only to the patient but to the Home Health staff
who frequently leave their homes and families to provide help, comfort, and
medical intervention to patients and families.
Memorials and donations are greatly appreciated.
A contribution is tax deductible and
may be of several methods. No individual
employee may accept a gift of value, for services. If the donor wishes to donate for the benefit
of all employees of Home Health, this must be stated when the donation is made.
